Action Plan and reintegration

Short answer

When there is an employer, the employee and employer draw up the Action Plan (Plan van aanpak) no later than 2 weeks after the Problem Analysis (Probleemanalyse). If you receive a Ziektewet benefit and have no employer, you make a plan with UWV’s reintegration adviser.

Step by step

    Read the Problem Analysis

    Check which functional abilities, limitations and recommendations the occupational physician (bedrijfsarts) has described. Medical details do not belong in the employer’s section.

    Make specific arrangements

    Record duties, hours, workplace adjustments, responsibilities, review dates and achievable next steps.

  3. 3

    Review progress regularly

    According to UWV, the employer and employee discuss progress at least once every 6 weeks and adjust the plan where necessary.

    Report problems early

    If arrangements break down, discuss this with the case manager or reintegration adviser. An expert opinion (deskundigenoordeel) may be available in some situations.

Deadlines and responsibilities

  • Employee and employer: jointly agree, implement and review the arrangements.
  • Occupational physician or occupational health service: provide the Problem Analysis and medical-functional advice.
  • Case manager or UWV adviser: monitor progress under the applicable procedure.

Do not wait while a deadline is running

Do not sign without checking the document. Record factually what you disagree with and request a correction or explanation in good time. For a UWV plan, the decision states whether an objection (bezwaar) is possible.
